Repeat prescriptions & 12-month prescriptions: what changed & how it works

Confused about a repeat prescription in Ireland and the new 12 month rules? This guide explains what changed, when a pharmacist can extend a 6 month script, and how costs work under State schemes.

What changed, 12 month prescription validity in Ireland

Since 2024 the maximum legal validity of many prescriptions has been extended to 12 months. Prescribers can write up to 12 months. In some cases a pharmacist may extend a 6 month prescription to a total of 12 months after safety checks. See the official update from the Department of Health and the Pharmaceutical Society of Ireland.

If you are due a renewal, our guide to ordering a prescription online shows how to request repeats and arrange delivery.

Who can extend a repeat prescription

Pharmacists may extend prescriptions when your treatment is stable, you are tolerating your medicine well, and it is appropriate to continue without a dose change. We review the original issue date and clinical suitability before deciding.

Extensions are not automatic. If your medicine needs a GP review or closer monitoring, we will explain the quickest way to renew with your prescriber.

What pharmacists check before extending

We ask about your current dose, side effects, interactions and any recent health changes. We also confirm supply dates and ensure the total duration does not exceed the legal maximum.

For step by step help with renewals, see how to order a repeat prescription online or contact us.

What usually cannot be extended

Short courses for acute conditions, medicines that need frequent dose adjustments, or treatments that require close supervision generally cannot be extended at the pharmacy. Controlled drugs and items with specific monitoring requirements typically need a prescriber review.

Save with 3 month or 6 month prescription orders

Where clinically appropriate and permitted by your prescription, you can choose a 3 month or 6 month supply in one order. This can simplify refills and may reduce the overall per month cost. We show clear prices for 3 month and 6 month supplies on our prescription prices page.

If you are unsure what supply length suits your medicine, ask the pharmacist. We will confirm what is allowed on your script and outline the most cost effective option.
